What is Penile Curvature?
If erection of a penis is not a verticalin the right, left, upward or downward and curvature, this is called penile curvature. In a normal person, when the penis has an erection, the penis should stop at an angle of 90 degrees with the body.
What are the Causes of Penile Curvature?
Penile curvature may occur congenitally or may acquire later.
Congenital penis curvature: Normally, there are three cylindrical structures inside the penis. These are two spongy tissueswhich provide the erection of the penis by blood supply and called corpus cavernosumand another spongy tissue in which urethra passes through. When there is growth retardation in one of these spongy structures in the mother’s womb, the penis is inclined towards the underdeveloped side.
In addition, hypospadias disease, which is a congenital disease and known as half circumcision among the population, the urinary meatus is located below where it should be. In this disease, when the penis erects, curvature occurs.
Acquired Penile Curvature:The most common cause of acquired penile curvature, which occurs later, is Peyronie’s disease, especially in older age. Peyronie’s disease is a disease that occurs as a result of calcification of the outer sheath surrounding the penis tubes that make the penis erected. In Peyronie’s disease, which usually occurs after the age of 50 years, there is pain in the penis, curvature of the penis and erection during erection. Penile surgery and penile injuries may also cause penile curvature.
During theerection, the normal posture of the penis is slightly upward and flat. Penile curvature of the penis is mostly upward curvature, sometimes the curvature can be to the right or to the left and sometimes to the downward.
What Are the Risk Factors of Penile Curvature?
Penis fractures during sexual intercourse may cause penile curvature. The incidence of Peyronie’s disease is higher in diabetic patients. In some connective tissue diseases, Peyronie’s disease is more common.
What is the Treatment of The Penis Curvature?
The treatment of penis curvature depends on the underlying cause. Penile curvature may be caused by hypospadias. Treatment of hypospadias disease is surgery. In this surgery, when the penis reaches a sufficient length, the urinary meatus is moved to the tip of the penis before the child reaches the age of adult and the curvature of the penis is regenerated.The most common cause of penis curvature in advanced ages is Peyronie’s disease. The treatment of this disease depends on the stage of the disease. In the early period of the disease, the plaque that causes penile curvature is not yet fully developed and therefore drug treatment is applied.
In the later period of the disease, when the painful period ends and deformity begins, there is no effect of drug treatments. In this period, some substances can be injected into the stiffness of the penis or shock therapy can be tried. However, the treatment to be applied in this period is the correction of curvature by surgery.
In cases of curvature of the penis due to reasons other than that, if the degree of curvature is above 30 degrees and the curvature that prevents sexual intercourse, surgery is recommended.
The most important problem seen after penile curvature surgery is the recurrence of penile curvature. The most important reason for this is the stitching of the sutures due to erections in the night, especially in adult patients. Therefore, in the early postoperative period, a number of medications are used to prevent night-time erections. There should be no sexual intercourse during the 4-6 week period.
